  1. Have Customer sign "3rd Party Form" to have shop manage DOI complaint on customer's behalf
  2. Collect Supporting Documents that Violate "Unfair Claims Practices"
  3. Fill out Information on Form
  4. Submit to IABA's Government Relations Manager" for review and Modifications at jack@mgrco.org
  5. Submit to DOI after approval via email at consumerservices@idoi.in.gov
